 ices  present a free six week workshop called &ldquo\;My Life\, My Healt
 h.&rdquo\; This  program is offered to adults who are living with\, or c
 aring for someone  with one or more persistent health conditions. Partic
 ipants can learn  how to set and meet personal goals\, identify and try 
 out simple  techniques that can improve your quality of life\, improve c
 ommunication  with your doctor\, handle stress and learn to relax\, incr
 ease your energy  level\, make choices about your treatment\, and have f
 un!\nThe Chronic Disease Self-Management Program was developed by  resea
 rchers at Stanford University Medical Center and has been proven to  hel
 p people just like you! Participants will receive a copy of the book  &l
 dquo\;Living a Healthy Life with Chronic Conditions\,&rdquo\; and a rela
 xation CD.\nSessions will run from 1:00 pm &ndash\; 3:30 pm and refreshm
 ents will be provided.
